Registration for the Lactations Room Services

How to Register:

To register for the UCSD Lactation Services Program please complete the Lactation Services Program Registration Form and return it to the Women’s Center.
You may e-mail:  women@ucsd.edu, fax: 858-822-1577, or send the form to the Women’s Center, MC:  0096.

Once we have received the completed form we will immediately mail you an information packet that will include the code required to access your chosen lactation facility.  Please contact Jessica Chapin-Geipel at jlchapin@ucsd.edu or 858-822-1479 if you have questions.
